Ensure that all fields are filled out, the application is signed and dated, and all necessary documents, including transcripts and letters of recommendation, are attached. • 14. List dual-credit/college accredited classes: Provide a list of any dual-credit or college-accredited classes the applicant has completed or is enrolled in.
• 15. Have you worked part-time during your school year?: Indicate if the applicant has worked part-time during the school year. If so, provide details such as where employed, primary duties, and dates of employment.
• 16. List hobbies or interests: Provide a list of the applicant's hobbies or interests.
• 17. What college, trade school or university will you be attending?: Specify the college, trade school, or university the applicant plans to attend.
• 18. What are your reasons for selecting this college, trade school or university?: Explain the applicant's reasons for selecting the specified educational institution.
• 19. What major course of study do you intend to pursue and why?: Indicate the major course of study the applicant intends to pursue and provide a brief explanation of why they chose this field.

If you fail to submit the 2024 PMT FOUNDATION Scholarship Application by the deadline, you will not be considered for the scholarship. Missing the deadline or submitting an incomplete application may result in disqualification and the inability to receive financial assistance.
